| How to use the "type-in" versions of our pdf forms |
| There are 2 ways to use the "type-in" versions of our pdf forms. Please read the
following instructions carefully. Click the link titled "Continue" at the end
of these instructions to proceed to the form. |

In browser mode, you can type directly into the form. Click on any field
and type the data. If it’s an OCR field, the characters you enter will
automatically be placed within the boxes as soon as you move or tab to the next
field. Special buttons are available on page one of the form to print the
entire form or reset all fields. To be able to view the form online, type in
the fields on the form and to print a hard copy of the form you will need
to have Adobe Reader installed (a free download is available on
the Adobe web site). If you want to be able to save a copy of this form
with data entered in the fields, the Adobe Reader will not be sufficient. Instead, you need to have the full version of Adobe
Acrobat on your computer.

You can save a PDF copy of this form locally and work with it in a similar
manner interactively on your own computer. This may be particularly useful if
you need to file more than one form and if you need to save an electronic copy
of this form on your computer. In addition, in order to enter or change data on
a local copy of the form, you must have the full version of Adobe Acrobat
installed on your computer. |

Please Note:
- This online form is not
linked to the SLD’s databases, so no fields get pre-populated and there are no
logical tests of data consistency. It is designed simply for easier data entry
— easier, that is, than hand printing.

If you attempt to save a completed form locally while using only the Adobe Reader
your data will be lost when you open the form again. You must be using a full
version of Adobe Acrobat to be able to save it
locally.
