| The table below provides links to the entity numbers for schools and libraries
in the SLD database as of November 13, 2000. If you represent an eligible
school or library and cannot find an entity number for your school or library
in the lists for your state or territory, please contact the SLD's Client
Service Bureau at 888-203-8100.
The entity number files are listed by state or territory. For each state or
territory, there are up to three files, one for public schools and school
districts, one for libraries, and one for other entities. The files are in
Microsoft Excel format. More information about each type of file is included
below:

The Public Schools and School Districts file lists the name and entity
number for school districts in that state in the SLD database. For each
district, the schools in our database related to that school district are
listed, along with its entity number. Both districts within a state and schools
within a district are listed alphabetically. If there are schools missing from
a particular district, or districts missing from a particular state, first
check the "other entities" file for that state. If the schools are not listed
in either the schools and districts file or the other entities file for your
state, contact the Client Service Bureau at 888-203-8100.

The Libraries file contains an alphabetical listing of the library
entities in a state, along with the entity number. This list includes the
library outlets/branches that are included in our database as library entities.
If there are libraries missing from a particular state, first check the "other
entities" file for that state. If the libraries are not listed in either the
libraries file or the other entities file for your state, contact the Client
Service Bureau at 888-203-8100.

The Other Entities file for each state includes the other entities that
we have listed for that state which are not affiliated (in the SLD database)
with a particular school district or listed as a library. Entities included in
this file include private schools, some consortia, and others. If an entity
that should be listed in the other entities file is not included, please
contact the Client Service Bureau at 888-203-8100.
