Form 470 and Form 471 Information for FY 2010
During the SLD’s IT Upgrade

Many of the SLD’s online interactive filing and databases resources will not be available during the SLD’s system update period.  These resources became unavailable at 5:00 p.m. DST on September 1, 2009, and are targeted to remain unavailable until 9:00 a.m. DST on September 14, 2009.

Form 470 Information During the Upgrade:

To provide Form 470 information for service providers in the interim, the SLD is maintaining a basic list of all Form 470s filed for FY 2010 since July 21, 2009.  This list is available at:
http://www.universalservice.org/sl/tools/search-tools/forms-470/Forms-470-table.aspx.

The list includes applicant names and state locations, Allowable Contract Dates, and Yes/No indications of service categories requested.  A link to an online PDF copy of each Form 470 is also provided.  Both the list and the PDF files will be updated for any “visually-correct” paper Form 470s submitted during the IT transition period.

The SLD had suggested that interested parties download cumulative Form 470 data files prior to the system shutdown.  Such files include more detailed information in spreadsheet format on all Form 470s filed from May 1, 2009, through August 31, 2009.  Since this download facility is no longer available, we are posting static versions of the cumulative Form 470 files.  Please note that these files will not include any Form 470s filed on September 1, 2009, or thereafter.

The following files are available:
1. Three raw comma-delimited download files: statecum.txt, blk2cum.txt, and blk4cum.text.  Data fields for the basic “statecum” file: format.txt.
2. An Excel version of the “statecum” file, edited to eliminate duplicates and less important information, rearranged in a more logical manner, and expanded to include PDF links (if available): Form 470s for FY 2010.xls.
3. An Excel version of “blk2cum” file, showing details on services: Form 470 Block2s for FY 2010.xls.

Form 471 Information During the Upgrade:

The SLD’s Automated Search for Commitments (“ASC”) tool (see http://www.universalservice.org/sl/tools/search-tools/search-commitments/) is still working, but the more extensive and useful Data Retrieval Tool (DRT”) is not.

More user-friendly versions of the DRT information are, and will remain, available on the E-Rate Central Web site.  This information is current as of August 31, 2009, but cannot be updated until the SLD’s DRT becomes available again.

All users of the E-Rate Central Web site have access to funding data by state (and by BEN or SPIN) using the Funding Quick Search tool available in the upper left-hand corner of each State Information page.
